 What Is A Medication Review Consultation?

Not sure whether your medications are optimized and fully compatible with your medical conditions? Do you have multiple physicians in your circle of care including specialists and you're not sure whether all are fully aware of the medications you are taking? Our Medication Review Consultations are fully virtual (video/phone call) and the pharmacist will contribute to your circle of care with other healthcare providers and play an active role in your medication treatment plan.

Who Can Benefit The Most From A Medication Review Consult?

Our Medication Review Consultations are suitable for anybody taking any number of medications. However, the following patient groups would benefit the most from this service:

1. Home-bound patients who do not have regular access to a primary care provider.

2. Patients recently discharged from the hospital with changes in their medications.

3. Patients with complicated treatment regimens that take numerous medications (i.e. diabetic patients)

4. Patients who are confused about their medications, and do not fully understand the need for their medications or the way they work.

5. Patients experiencing side effects from their medications/are non-adherent to their medication regimen.

6. Patients who are not experiencing full benefit from their medications (ineffective treatments).

What Does A Medication Review Consult Achieve?

1. Ensures medication compatibility and catches and resolves drug-drug, drug-food and drug-medical conditions interactions.

2. Ensures medications and their doses are optimized (e.g. catches and eliminates duplication of therapy).

3. Focuses on eliminating medications that are no longer needed or unnecessary (deprescribing).

4. Explains the ins and outs of the medication treatments, when and how to best take them, and reduces the chance of potential side effects.

How Does It Work?

We have structured our Medication Review as a series of two reviews - an initial session and a follow up session as described below. 

​

The initial session serves to gather information about your health and medication history and for us to learn more about you. At this point we will look through your medication regimen for interactions, discuss potential side effects and look for opportunities to eliminate duplication of therapy/deprescribe. We will use the information gathered during this session to make recommendations to your healthcare team to streamline your drug therapy.

In the follow up session we will discuss our recommendations, map your treatment journey within the circle of care, and review/provide further guidance on how to implement these recommendations into your lifestyle. ​

How To Book?

For your initial session from our booking system you can indicate one of three times that would work best for you. Please provide a time 72 hours or more in advance of your booking date so that we can prepare for your appointment accordingly. We will confirm your appointment and you will receive a confirmation email. For a video appointment you will use the link in your confirmation email to join the zoom call. If you have selected a phone appointment your consultant pharmacist will call you.

Once we have heard back from the rest of your healthcare team in regard to our recommendations from the initial session we will be in touch to schedule your follow-up appointment.
